Health and care staff are invited to take part in a short questionnaire to help improve how the system supports Inclusion Health groups.

Insights from this survey will contribute to safer, more inclusive services, helping to reduce missed appointments and unnecessary A&E visits, and improving outcomes for people often left behind by mainstream care.

This is a short survey and should take no longer than ten minutes to complete.
